New Patients

 We are honored to serve veterans and their families by providing high-quality primary care, behavioral health services, and veteran-focused medical evaluations. Our goal is to make the process of establishing care as simple and convenient as possible. 

 Book your appointment online through our secure patient portal or contact our office for assistance. 

 After scheduling, you will receive access to our secure patient portal where you can complete your new patient paperwork, medical history forms, and consent documents prior to your visit. 

 Depending on the type of appointment, you may be asked to upload:

•  DD214
• Government-issued photo ID
• Insurance card(s)
• Current medication list
• Previous medical records
• Military or VA records (for veteran evaluation services)
• Past VA Decision Letters

 Appointments may be conducted in person or through our secure telehealth platform. Our provider will review your medical history, discuss your concerns, and develop a personalized care plan. 

 After your appointment, you can access documents, communicate with the clinic, and schedule follow-up visits through the patient portal. 

We accept self-pay patients and participating insurance plans. Copays, deductibles, and out-of-pocket costs vary by insurance provider.
