Our goal is to make the process of establishing care for everyone healthcare and accessing primary care services as simple and convenient as possible, while also integrating essential behavioral health services.
Step 1: Schedule an Appointment
Book your appointment online through our secure patient portal for our behavioral health services, primary care services, and veterans healthcare.
Step 2: Complete Your Registration Forms
After scheduling your appointment for our primary care services, you will gain access to our secure patient portal. Here, you can complete your new patient paperwork, medical history forms, and consent documents, which are essential for both behavioral health services and veterans healthcare.
Step 3: Upload Relevant Documents
Depending on the type of appointment for your behavioral health services or primary care services, you may be asked to upload: DD21, a government-issued photo ID, your insurance card(s), a current medication list, previous medical records, military or VA records for veterans healthcare evaluations, and past VA Decision Letters.
Step 4: Attend Your Appointment
Appointments can be conducted in person or through our secure telehealth platform.
Step 5: Follow-Up Care
After your appointment, you can access your documents, communicate with the clinic about your behavioral health services, and schedule follow-up visits for both primary care services and veterans healthcare through the patient portal.
Insurance & Payment
We accept self-pay patients and participating insurance plans for our behavioral health services, which also encompass primary care services for veterans. Please be aware that copays, deductibles, and out-of-pocket costs may vary based on your insurance provider.
